Aeon 5.1 added the unlimited custom fields feature but did not increase field sizes.

Most sites require patrons to fill out the State field. The State dropdown includes an N/A option. Aeon should include an additional database field for Province/State for international researchers. If possible, this field should be conditional and only appear if the N/A option is selected from the State dropdown.

Would also be helpful to update the history entry recorded when an item is uploaded. It currently just says "request information modified" and gives no indication to staff whether an item has already been uploaded.

I should say for the above, when a staff person clicks the New Request for User button, the scheduled date field should populate with the current date. Right now, it's just blank by default.

Sites would like the ability to both send customized permission forms (maybe via email attachment or new print template) and allow patrons to return signed forms. Allow form to be visible to both patron and staff. Potentially will require more fields on request to capture the necessary information related to permissions (ex: publication date, publisher, etc).
Yes. I tried to add SiteDescription to the ViewDetailedManuscriptInformation and ViewDetailedMonographInformation pages, but only site code can display there. It'd be great if it could display consistently across all the web forms.
