Cover sheet for Lending Requests
Add the ability to apply a coversheet on the lending side which would allow lenders to include copyright information as required by the CONTU guidelines.

We need to add copyright/licensing statements to articles we send to other libraries. Technically we are not allowed to send them without this. We can do this easily for scanned from print articles, but not anything delivered via the Electronic Delivery Utility (these are usually downloaded from e-resources). We really need a way to add this coversheet automatically (RapidX can do this!).
