Gray out notification methods not selected
Gray out Email Notifications and SMS Notifications checkboxes (on the User form) if the selected Notification Method is Mail or Phone

The typical reason to "gray something out" on the web is to show it is no longer a possible option (one cannot select it)--in this case, it seems that if they can still express their preference by selecting a new option then it shouldn't be grayed out.
