Purdue University would like this feature. They would like for students to submit an appointment to view class materials. These materials are checked out to the activity. They would like for dedicated fields on the appointment form where the student can write in the name of the course and the name of the instructor, and also maybe a checkbox that denotes that the appointment is for a class (activity) as opposed to a regular reading room appointment.

I left the title of this request to be more generic about the availability of "custom fields" on the appointment request form. New York State Archives would also like this feature. They will be integrating inter-library loan processes in Aeon, and ILL has a limit of 5 microfilm rolls (which could be recorded in the page count field) per 4-6 weeks.

Purdue University allows for patrons to make new requests during their reading room visit/appointment, and would also like this feature.

The University of Washington would also like this feature. One of their reading rooms is a virtual reading room, and the patron can only have 1 "item" at a time for the virtual reading room appointment.

The University of Washington would also like this feature. They are using a custom field to record preferred pronouns. They would like for this field to display on the signed-in users grid.

Purdue University would also like this feature. They would like for the researcher to be able to select multiple collection areas of interest during registration. On the web registration form, the controlled vocabulary options are presented in a multi-select checkbox list. If staff need to modify anything on the staff side, though, they cannot select multiple options from a controlled vocabulary list.
