Allow for custom fields to be added to patron appointment request
California State University L.A. would like to have a drop-down field on the appointment form for the patron to choose the purpose of their appointment. They don't want the field to be on the request form, because the patron would have to select the purpose for each item request for that appointment if the field was required. And they have many returning researchers, so they don't want the field to be on the user registration, as it is more specific to the appointment/reason for the visit.

Purdue University would like this feature. They would like for students to submit an appointment to view class materials. These materials are checked out to the activity. They would like for dedicated fields on the appointment form where the student can write in the name of the course and the name of the instructor, and also maybe a checkbox that denotes that the appointment is for a class (activity) as opposed to a regular reading room appointment.
